Heat relaxes the muscle, and gets blood moving where it's needed
Period pain comes from prostaglandins making the womb tighten, squeezing off its own blood supply. Warmth relaxes that muscle and pulls more blood into the area, easing the squeeze, spasm, and short-term lack of oxygen that actually cause the ache.
